Order Name Tags
It is easy to order you custom name tag! Just fill in the 'Order Information' box below and for each name tag ordered tell us:
- Material - plastic ( wood - temporarily unavailable)
- Color - red, blue, pink, green, yellow, gold, silver
- Fastener type - pin or magnet
Each name tag is $5.00 and they are custom made.
You will be contacted by phone (916-712-9885) or email (pb.nametags@yahoo.com) when they can be picked up.
After you have ordered a name tag you may need to check your spam folder for the email notification that it is done.
