Name Tags

  

It is easy to order you custom name tag!  

 

But time is running short! Last day to order is April 26th. Last day to pick up is April 29th.

Name tags will return in the fall.

 

Just fill in the 'Order Information' box below and for each name tag ordered tell us:

  • Color - tropical pink, soft pink, green, yellow, silver, blue, gold, and red

  • Fastener type - Magnet recommended, Pin if using with a visor.

  • All name tags are plastic.

 

Each name tag is $5.00 and they are custom made.
 
You will be contacted by phone (715-491-6497) or email (swanson.kristie@gmail.com) when they can be picked up.

After you have ordered a name tag you may need to check your spam folder for the email notification that it is done.
